Below is information for your first visit.
LOCATION
We are located at the CPMC Davies Campus in San Francisco. Our office is in the Medical Office Building at 45 Castro Street. This is located at the corner of Duboce Ave and Castro St and is “up the hill” from the main hospital’s emergency department entrance as shown on this map of the campus. After entering the 45 Castro Street Medical Office Building, our office is on the first floor to the right of the elevators. As you go down the long hall, the clinic entrance is the second door on the left at Suite 121.
PARKING
Street parking is available, but visitors without a neighborhood permit are limited to two hours. It often takes a while to find a parking spot so please arrive early if you plan to park on the street. For a fee, parking is available at the on-site garage. There are also several public transportation routes available including the N Judah and multiple bus lines.
WHAT TO BRING
If you have received care elsewhere, it is very helpful if you could bring any relevant records. This particularly includes “Operative Reports” from prior surgery, any imaging (x-rays, CT scans, MRI, etc) including both the printed report from the radiologist and a copy of the actual images, and nerve conduction study reports (“EMG/NCS”).
